Who we are?
South Brisbane Aged Care Services, based on Star Medical Centre – Algester/Woodridge, is one of the biggest GP service providers for Aged Care Facilities in Brisbane. We specialize in Aged Care GP services. Currently, our team looks after about 700 residents, including 14 nursing home and 2 retirement villages. Our principles: passion, cooperation, quality of services, holistic approach and multidiscipline.
Our team includes 5 experienced GPs with strongly passionate about caring for elders, 2 practice nurses. We are confident about how to care for the residents, how to communicate with the residents’ family and how to make the facilities staff life easier.
What services we provide?
- Individualized regular clinics: clinics in facilities are conducted by discussing with facilities management and clinic staff in term of day and time. We understand the importance of flexibility, efficiency, effectiveness and cooperation between different teams.
- Minor procedures: most GP clinic procedures may be provided on site in facilities ( which depend on the facilities setting and policy), including biopsy, skin cancers excisions, other skin lesion excisions, toe nail removal, ear syringe, cryotherapy, ECG and ABI etc.
- Practice nurse services: ECG, ABI and some other services can be done by our practice nurses and organized through the GPs.
- Multidiscipline approach: we work closely with other allied health: physiotherapist, dietitian, speech therapist, pharmacist, psychologist/mental health nurses, podiatrist, audiologist, and optometrist, and also work closely with hospital and community palliative care team, dementia outreach team, older person mental health team.
